Fungal nails can be more complex than they appear. The fungus invades the nail and can thrive, on top, in the nail and primarily beneath it in the nail bed. Effective treatment requires addressing the fungus at the nail bed or as close to it as possible.

Treatment Options
Fungal nail infections are common and can be persistent, but with the right assessment and treatment plan, they can be effectively managed. At your initial appointment, we will carefully assess the affected nail or nails, discuss your concerns, and recommend the most appropriate treatment option for you based on also carrying out a fungal nail test to check if it is truly fungal. Clinical photographs may be taken to help monitor progress over time.
We take time to explain each treatment option clearly, including expected outcomes, benefits, and any potential risks, so you can make an informed and confident decision. If appropriate, a nail sample may often be taken for testing through us using a “5 minute fungal test” or via your GP if we want to determine suitability for oral antifungal medication, such as terbinafine tablets, which are not suitable for everyone.
Conservative Treatment: Nail Reduction & Antifungal Lacquer
For many patients, treatment begins with significant reduction of the affected nail, allowing antifungal products to work more effectively. A clinically proven antifungal nail lacquer is then applied and advised for home use.
This treatment works best when combined with regular nail reduction appointments every six weeks, alongside weekly home care using nail files to keep on top of nail thickness ensuring the lacquer works optimally , we will guide you on how to use correctly. Many over-the-counter products do not contain effective antifungal ingredients, so using the correct, professional treatment is essential.
Advanced Treatment: Lacuna® / Clearanail®
Lacuna® and Clearanail® treatments are designed to target fungal infection more directly. Tiny channels are created through the nail plate, allowing antifungal spray to reach the area beneath the nail where fungal infection is most active.
This method improves penetration of antifungal treatment and can be an effective option for patients who have not responded well to topical treatments alone.
Advanced Treatment: Fungal Nail Removal
In cases of severe or long-standing fungal infection, nail removal may be recommended.
- Nail removal without phenol allows the nail to regrow. While regrowth is expected, we cannot guarantee the new nail will be completely unaffected. Healing typically takes around one week, after which antifungal treatment can begin on the exposed nail bed. Full nail regrowth usually takes approximately 12 months.
- Nail removal with phenol is a permanent option for patients who wish to prevent the nail from growing back. This is often chosen for recurrent or severe fungal infections, as it removes the risk of future reinfection in that nail.
